MyAntoMiH was conceived by a medical doctor with more than 40 years in practice to better help his patients take control of their total health care. As patients rely on the health care provider to maintain records and a health history, these organizations don’t always effectively communicate with one another. MyAnatoMiH allows you, the user, the patient, to maintain your own health history, your child, spouse, or parent.

MyAnatoMiH puts you in control of accessing and maintaining records from your primary care physician to specialists, to your dentist, OB-GYN, ophthalmologist, nutritionist, physical therapist, etc…  You know your health care providers do not all use the same record keeping systems and to copy records you may have to jump through hoops to gather all of your information in one place. MyAnatoMiH does this for you.

The first time you enter your information may take some time, however once the information is entered you only have to update as you experience health care issues. All of your health information is at your fingertips. You can easily access your information from your laptop or desktop computer, a tablet or a mobile phone. The information is secure in a server. It is not accessible by big data companies.

What is in the application

 Clinical Data

Use the My Clinical Data to search and enter your health history under medical diagnosis, traumatic injuries, hospitalizations, and surgeries and procedures. You may use the Wizard option to walk you through each section or enter the data manually.

Use this section to note actual medical conditions with which you have been diagnosed. Some examples of these are Type II Diabetes, High Blood Pressure, Acid Reflux, in the search area type in the name of your diagnosis, select it and enter it in your AnatoMiH record.

Use this section to note incidents of major trauma; this can include blunt or penetrating injuries, falls, auto accidents, gunshot wounds, major burns. Traumatic injuries may have the potential to cause prolonged injury or death. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

Use this section to note when and where you have been hospitalized due to injury, accident, or critical health needs. Enter the name and address of the hospital, attending physicians, and the nature of your stay.

Use this section to note in-patient and out-patient instances where you have undergone a surgery or procedure to correct a medical issue or diagnosis. Some examples of common surgeries or procedures are appendectomy, breast biopsy, coronary artery bypass, and so forth. Type the name of your surgery or procedure in the search area and select each one to add to your health history.

 Specialties

Use the My Specialties Section to search and enter your health history under OB/GYN and Men’s Health, Dental Health, Eye Health, and Mental Health specialties.

Use this section to enter health information that relates to obstetric and gynecological diagnosis and procedures. This can relate to childbirth, miscarriages, breast exams, hormone treatments, or other issues related to women’s health and wellness. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

Use this section to enter health information that relates to men’s health diagnosis and procedures. This can refer to prostate exams, ED, testicular diagnosis, and other issues related to men’s health and wellness. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

Use this section to enter information related to your dental care. This can include cavities, fillings, oral surgeries, root canals, dental implants, gum grafts, and other issues related to oral and dental health. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

Use this section to enter information related to your eye care. This can include glasses or contact prescription, cataract surgeries, glaucoma, macular degeneration, eye injuries, or other issues related to your eye care history. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

Use this section to enter information related to your mental health. This can include diagnosis of depression, ADD/ADHD, anxiety, eating disorders, or other issues related to your mental health history. In the search area type in the name of your diagnosis, select it, and enter it in your AnatoMiH record.

 Medications

Use the My Medications Section to search and enter your health history related to Prescriptions, Over-the-Counter-Drugs, Substance Use, Allergies, and Drug Reactions.

Use this section to enter information related to current and past medication prescribed to you. You may enter the drug name, the prescribing physician, the pharmacy you use, and the health condition related to the prescription. For example, if you are diagnosed with high blood pressure, you may list the blood pressure medication you use and note that it is to treat your high blood pressure. You may also track your blood pressure readings in this area.

Use this section to note the vitamins, supplements, or medications you routinely take. You may enter the name of the item and the health condition for which you are taking the OTC.

Use this section to note the use of non-prescription or non-over-the-counter substances. This could include CBD or THC products, opioids, alcohol, or other substances.

Use this section to note any allergies, including seasonal or food allergies such as hay fever, pollen sensitivities, peanut allergies, shellfish allergies, etc. If you react to specific prescriptions such as penicillin or other medications, please enter the information here. You may include the date of the first reaction, type of response, swellings, rash, etc. and if you used a remedy such as an EPI pen or other treatment.

 Historical Data

Use the My Historical Data section to search and enter your health history related to any childhood illnesses, immunizations, lab, and test results, and family history of health issues. You may also upload legal documents under the Family History section that may include a power of attorney, parental authorizations, wills, DNRs, or other documents related to health histories and procedures.

Use this section to enter any childhood illnesses you may have had. This can include chickenpox, mumps; health issues experienced infancy – such as premature birth.

Use this section to enter your immunization history from the standard series of childhood immunizations to flu shots to travel vaccinations with dates and supervising clinician.

Use this section to note and or upload test results, mammographies, x-rays, blood pressure or diabetic tracking, blood tests, or other information related to lab work, images, or test results. This information may be uploaded as PDF files.

Use this section to upload genetic testing information or family health histories. This may include parental and maternal health histories such as cancer, heart disease, high blood pressure, diabetes, or other hereditary diseases or conditions that may influence your personal health history or diagnosis.
